Humana Inc. is seeking a Manager, Care Management to lead Stars-focused associates in a remote role. You will guide outreach, triage member needs, and drive care-gap closure while aligning with enterprise priorities and quality standards.

Reporting to the Associate Director of Care Management, you will coach teams, monitor performance, and promote accountable, member-centered care within a dynamic healthcare ecosystem.

Qualifications

• Active, unrestricted RN license with no disciplinary action; compact licensure required.
• 5+ years of RN experience.
• 3+ years of case management experience.
• 1+ years of data-driven leadership experience, including analyzing performance and implementing improvements.
• Strong computer proficiency in Microsoft Office and ability to learn multiple software applications.

Responsibilities

• Provide leadership and coaching for associates conducting telephonic, Stars-focused outreach.
• Identify member needs beyond outreach and triage to appropriate clinical, social, or community resources.
• Set clear performance expectations and manage outreach, care-gap closure and productivity.
• Translate Stars priorities into team actions and monitor results.
• Collaborate with Enterprise Stars, quality, analytics and other teams to improve performance.
• Communicate on leadership calls and present recommendations.
