Job summary

Humana Inc. is seeking a Manager of Care Management to lead a team of Stars-focused associates delivering targeted telephonic outreach to close care gaps and connect members with resources. This remote role reports to the Associate Director of Care Management and drives improvements in quality, outcomes, and member experience.

You will guide performance, coordinate across enterprise teams, and ensure adherence to privacy and compliance while shaping Stars priorities and measures.

Qualifications

• Active, unrestricted RN license with no disciplinary action.
• 5+ years of RN experience.
• 3+ years in case management.
• 1+ years of data-driven leadership experience.
• Strong proficiency with Microsoft Office and multiple software applications.

Responsibilities

• Provide leadership and coaching for associates who conduct telephonic, Stars-focused member outreach.
• Identify needs beyond outreach and refer members to clinical, social, or community resources.
• Set clear expectations and manage performance across outreach, care-gap closure, productivity and documentation.
• Collaborate across enterprise partner teams to identify opportunities to improve performance and impact.
• Translate Stars priorities into clear team actions and monitor results to improve member engagement and care-gap closure.
