Job summary

Humana Inc. is seeking a Manager, Care Management to lead a remote team focused on Stars initiatives, closing care gaps, and connecting members with appropriate resources.

You will mentor associates, monitor performance, and drive member-centered outcomes within a data-driven environment. You will report to the Associate Director of Care Management and collaborate with Enterprise Stars, quality, and analytics teams to align outreach with enterprise priorities.

Qualifications

• Active, unrestricted RN license with no disciplinary action; compact licensure required.
• 5+ years of RN experience.
• 3+ years of experience in case management.
• 1+ years of data-driven leadership experience, including analyzing performance and implementing improvements.
• Strong computer proficiency and ability to learn multiple software applications.

Responsibilities

• Provide leadership and coaching for associates conducting telephonic, Stars-focused member outreach.
• Foster excellence in quality through feedback, reviews, and ongoing development.
• Manage performance across outreach, care-gap closure, productivity, and health outcomes.
• Translate Stars priorities into team actions and monitor results to improve engagement and outcomes.
• Collaborate with Enterprise Stars, quality, operations, analytics, and other stakeholders to optimize performance.
