Job summary

Humana Inc. seeks a Manager of Care Management to lead a Stars-focused team delivering targeted telephonic outreach, closing care gaps, and connecting members with resources.

You will guide associates to promote quality, accountability, and member-centered outcomes, reporting to the Associate Director of Care Management. You will influence performance, triage complex needs, and collaborate with enterprise partners to align outreach with Stars priorities and organizational goals.

Qualifications

• Active, unrestricted RN license with no disciplinary action.
• 5+ years of RN experience.
• 3+ years of case management experience.
• 1+ year of data-driven leadership experience with measurable improvements.
• Strong computer proficiency in Microsoft Office and ability to learn multiple software applications.

Responsibilities

• Provide leadership and coaching for associates conducting telephonic Stars-focused member outreach.
• Ensure associates identify needs beyond outreach and triage/refers to clinical, social, or community resources.
• Inspire an engaged, accountable team and set expectations on performance, quality, and compliance.
• Translate Stars priorities into clear team actions and monitor results to improve engagement and outcomes.
• Collaborate with Enterprise Stars, quality, operations, analytics, and other stakeholders to improve performance and impact.
• Support onboarding, training, and mentoring on Stars measures and workflows.
