Qualifications

• Active, unrestricted RN license required.
• 5+ years of RN experience.
• 3+ years of case management experience.
• 1+ year of data-driven leadership experience with measuring performance and implementing improvements.
• Strong proficiency with Microsoft Office and multiple software applications.

Responsibilities

• Provide leadership and coaching for associates conducting telephonic, Stars-focused outreach.
• Ensure identification of needs beyond initial outreach and triage/referral to clinical, social, or community resources.
• Set clear performance expectations across outreach, care-gap closure, productivity, and documentation.
• Translate Stars priorities into team actions and monitor results to improve engagement and outcomes.
• Collaborate with Enterprise Stars, quality, operations, analytics, and other teams to improve performance and impact.
