Job summary

Humana Inc. seeks a Manager, Care Management to lead a team of Stars-focused associates delivering telephonic outreach to close care gaps and connect members with necessary resources.

This remote role reports to the Associate Director of Care Management, guiding priorities and performance amid evolving Stars targets. You will drive team development, ensure high quality care coordination, and collaborate with enterprise partners to optimize member outcomes and program performance in a dynamic

Qualifications

• Active, unrestricted RN license with no disciplinary action.
• 5+ years of RN experience.
• 3+ years of case management experience.
• 1+ years of data-driven leadership experience, including analyzing performance and implementing improvement plans.

Responsibilities

• Provide leadership and coaching for associates conducting telephonic, Stars-focused member outreach.
• Ensure needs beyond initial outreach are identified and referred to clinical, social, or community resources.
• Inspire an engaged, inclusive, and accountable team by setting professional standards and recognizing contributions.
• Translate Stars priorities into clear team actions and monitor results to improve engagement and outcomes.
• Collaborate across enterprise partner teams to identify opportunities to improve performance and impact.
