Qualifications

• Active, unrestricted RN license, or LCSW or LCPC in Illinois.
• 5+ years of professional experience in healthcare and/or care management.
• 2+ years of leadership experience.
• Ability to use electronic medical records and analyze data trends.
• Willingness to travel 25% statewide.

Responsabilités
