Job summary

Humana Inc. is seeking a Manager, Care Management to lead a remote team of Stars-focused care coaches responsible for closing care gaps, identifying health and social needs, and connecting members with resources.

You will report to the Associate Director of Care Management and drive member-centered outcomes in a data-driven environment. You will coach associates, set performance standards, triage complex cases, and collaborate with Enterprise Stars, quality, operations and analytics to improve

Qualifications

Active, unrestricted RN license with no disciplinary action.
5+ years of RN experience.
3+ years of case management experience.
1+ year of data-driven leadership experience.
Strong computer proficiency in Microsoft Office programs and ability to learn multiple software applications.

Responsibilities

Provide leadership and coaching for associates conducting telephonic, Stars-focused outreach.
Identify needs beyond the initial outreach and triage or refer members to resources.
Set and monitor performance across outreach, care-gap closure, productivity, and health outcomes.
Translate Stars priorities into team actions and monitor results.
Collaborate with Enterprise Stars, quality, operations and analytics teams.
