Humana Inc. is seeking a Manager, Care Management to lead a remote team focused on Stars initiatives. You will guide care coaching to close care gaps, identify needs, and connect members with clinical or social resources to improve health outcomes.

You will report to the Associate Director of Care Management and collaborate with enterprise partners to optimize performance, quality, and member experience across the Stars program.

Qualifikationen

• Active, unrestricted RN license required.
• 5+ years of RN experience.
• 3+ years in case management.
• 1+ year of data-driven leadership experience.

Aufgaben

• Provide leadership and coaching for associates conducting telephonic Stars-focused outreach.
• Ensure needs beyond initial outreach are identified and triaged to appropriate resources.
• Inspire an engaged, accountable team; set expectations and manage performance.
• Translate Stars priorities into actionable team plans and monitor results.
