Humana Inc. seeks a Manager, Care Management to lead a remote team of Stars-focused Care Coaches. The role focuses on telephonic outreach to close care gaps, identify health and social needs, and connect members with resources to improve health outcomes.

You will guide priorities, coach associates, and ensure high-quality, compliant care aligned with Stars measures. This role reports to the Associate Director of Care Management and requires RN licensure and data-driven leadership.

Qualifications

• Active, unrestricted RN license with no disciplinary action.
• 5+ years of RN experience.
• 3+ years of case management experience.
• 1+ years of data-driven leadership experience, including analyzing performance and implementing improvements.
• Strong computer proficiency with MS Office and multiple software applications.

Responsibilities

• Provide leadership and coaching for associates conducting telephonic, Stars-focused member outreach.
• Ensure associates identify needs beyond initial outreach and triage to appropriate resources.
• Inspire a high-performing, inclusive team and set expectations across outreach, care-gap closure, and health outcomes.
• Translate Stars priorities into clear team actions and monitor results to improve engagement and outcomes.
• Collaborate with Enterprise Stars, quality, analytics and other teams to optimize performance.
