Humana is seeking a Field Care Manager, Behavioral Health to perform face-to-face and telephonic assessments and coordinate integrated behavioral health care for Illinois members. You will lead an interdisciplinary care team, collaborate with providers, and ensure timely, high-quality services while navigating regulatory requirements.

Remote role with up to 75% travel; hours Mon–Fri, CST. Requirements include IL license (LCSW/LMFT/LCPC) and 2+ years of post-degree clinical experience;

Formación

• Must reside in Illinois.
• Active Illinois license in LCSW, LMFT or LCPC (No supervisees or provisional licenses).
• 2+ years of post-degree clinical experience in behavioral health setting.
• Case management experience with complex SMI, SUD, SED.
• Ability to travel to region-based facilities and homes for assessments.
• Proficient with electronic medical records and Microsoft Office.
• Valid driver's license and transportation.

Responsabilidades

• Utilize behavioral health services through care coordination, crisis intervention, peer support, and collaboration with providers.
• Provide comprehensive support to members with behavioral health conditions across age groups.
• Engage members in their communities to build trust and coordinate care.
• Coordinate behavioral health and medical services with provider engagement and treatment plans.
• Improve health literacy and address health-related social needs.
• Lead the ICT and oversee care planning, transitions and service delivery.
• Facilitate ICT meetings and ensure team communication.
• Support transition of care efforts and collaborate with internal and external partners.
