Job summary

Tufts Medicine Integrated Network seeks a proactive administrative professional to support care management referrals and coordination across ambulatory networks. You will triage referrals, collaborate with health plans, and ensure timely documentation in the EPIC system to support comprehensive patient care.

In this role, you will develop relationships with internal and external collaterals, generate routine performance reports, and assist in onboarding staff.

Qualifications

• High School Diploma or equivalent; 2 years clnical setting experience.
• Preferred: College degree or Medical Assistant Certification Attestation.
• 4 years of clinical setting experience and experience in human services field preferred.

Responsibilities

• Triage referrals in EPIC Compass Rose to Ambulatory Care Management.
• Maintain up-to-date referral work queues; ensure timely turnaround.
• Document in EPIC and dept databases accurately.
• Serve as subject-matter expert for care management referrals to teams and partners.
• Generate and analyze regular reports on referrals and program trends.
