Job summary

Talkspace is seeking a Care Coordinator to support patients through their care journey within our referral management team. This remotely-based role focuses on activation, engagement, and coordinating with Talkspace providers and facilities to ensure smooth transitions of care.

You will monitor early engagement, identify barriers, follow up on missed requirements, and document activities while collaborating with internal teams to deliver high-quality mental health support.

Qualifications

• 1–2 years of experience in care coordination or related roles.
• Strong written and verbal communication skills.
• Ability to manage multiple patients and competing priorities.
• Comfortable with technology and navigation of multiple systems.
• Ability to work remotely within EST/CT time zones.

Responsibilities

• Assist with patient activation and follow-up to ensure timely appointments.
• Monitor engagement and support the care journey during the first 30 days.
• Coordinate with providers and facilities for escalations or transitions of care.
• Maintain accurate documentation of outreach and care coordination activities.
• Collaborate with internal teams to remove barriers to care.
• Address billing issues to prevent missed appointment charges.
