Job summary

TimeDoc Health is hiring Bilingual Care Coordinators to support chronic condition patients through CCM. This is a phone-based, productivity-driven role spent primarily on the phone coordinating care and documenting interactions.

Remote work with a fixed schedule and daily productivity metrics is required. The role targets clinical professionals with CMA/RMA or LPN credentials, strong English communication, and EMR/EHR experience.

Qualifications

• Must have CMA/RMA or LPN with 1–3 years of experience.
• Active certification or multistate license for LPNs.
• Strong verbal and written communication skills in English.
• Comfortable being on the phone most of the day.
• Experience with EMR/EHR systems.
• Organized, reliable, and able to work independently in a remote setting.
• Spanish language skills preferred.

Responsibilities

• Conduct monthly CCM phone calls with the assigned practice.
• Make 30–50 outbound calls per day.
• Complete required patient interactions to meet CCM service goals.
• Educate patients on chronic conditions and care plans.
• Coordinate care with provider offices, pharmacies, and community resources.
• Identify gaps in care and social determinants of health.
• Create and update personalized care plans in the EMR.
• Accurately document all patient interactions.
• Maintain HIPAA compliance and patient confidentiality.
