The position involves monthly CCM calls, patient education on care plans, and collaboration with providers, pharmacies, and community resources. A CMA/RMA or LPN with active licensure and strong English communication is preferred.

Qualifications

• 1–3 years of CMA/RMA or LPN experience.
• Active certification or multistate LPN license.
• Strong verbal and written English communication.
• Comfortable speaking on the phone most of the day.
• Experience with EMR/EHR systems.
• Organized, reliable and able to work independently remotely.

Responsibilities

• Conduct monthly CCM phone calls with the assigned practice.
• Complete required patient interactions to meet CCM service goals.
• Educate patients on chronic conditions and care plans.
• Coordinate care with provider offices, pharmacies, and community resources.
• Identify gaps in care and social determinants of health.
• Create and update personalized care plans in the EMR.
• Document all patient interactions accurately.
• Maintain HIPAA compliance and patient confidentiality.
