Job summary

Humana in Wisconsin is seeking a Manager, Care Coaching to lead and mentor the IDT staff across Jackson and Trempealeau Counties. You will partner with the Associate Director to ensure DHS contract compliance and deliver high-quality Family Care services.

This remote-eligible role requires a Bachelor's degree (or nursing associate) and 1 year of leadership in human services, plus 3 years in Family Care. Travel is expected about monthly, with a 40-hour work week and typical hours 8:00 am–4:30 pm

Qualifications

Bachelor's degree in human services or related field or associate degree in nursing.
1 year of leadership experience in the human services field.
3 years of experience working in the Family Care program.
Reside within 1 hour of either Black River Falls or Blair, WI.

Responsibilities

Mentor and support Field Nurses and Care Coaches to ensure DHS contract compliance.
Identify learning needs; arrange mentorship and ongoing learning for IDT staff.
Assess and coach staff on core competencies and performance standards.
Review reports, assess process and documentation.
Assess utilization and quality of care management for each member.
Participate in cross departmental collaboration to ensure quality and provider services.
Educate community partners.
Participate in hiring by selecting and training associates.
Facilitate in-person unit meetings and attend quality visits as needed.
