Job summary

Humana is seeking a Care Coach to assess member needs and guide them toward resources to achieve optimal wellness. This remote role focuses on coordinating care across clinical and community services while maintaining compliance.

The ideal candidate has at least an associate degree and 2 years of direct adult patient care, plus strong communication and system navigation skills. Training will be provided.

Qualifications

• Associate degree in any field required.
• Minimum 2 years of direct adult patient care experience.
• Experience interacting with healthcare team members.
• Strong computer proficiency and ability to learn various EMR systems.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess needs and develop individualized care plans.
• Coordinate services to clinical, community, and multidisciplinary resources.
• Identify and resolve barriers to care and independence.
• Monitor progress and adjust care strategies as needed.
• Maintain strict compliance in a regulated program.
• Navigate multiple computer systems while engaging members.
