Job summary

Humana is seeking a Care Coach/Service Coordinator to assess member needs and guide access to LTSS and community resources. The role includes meeting members in their location, building trust, and coordinating care with the care team to promote well-being.

Strong communication and organization are essential for success. Remote position with occasional travel for training or meetings; Indiana residency in specified counties is required, plus a valid driver's license and a private home office

Qualifications

• Bachelor's degree in health and human services field.
• 2 or more years of related experience.
• Use a variety of electronic information applications/software programs.
• Patient-facing role and commitment to consumer experiences.
• Valid state driver's license and vehicle liability insurance.
• Separate room with a locked door for privacy when working from home.

Responsibilities

• Administer LTSS assessments through person-centered thinking approaches.
• Contacts members telephonically or in-person to establish goals and priorities; develop plan of care and identify LTSS providers.
• Develop and modify Service Plan and involve care team members in care planning.
• Support members through navigation of LTSS and related environmental and social needs.
• Coordinate with Care Coordinator on referrals for non-capitated services and capture all services.
• Coordinate and consult with Humana-contracted providers regarding LTSS delivery.
• Participate in interdisciplinary Care team meetings (ICT).
• Connect and refer members to community resources and third-party payers.
• Assist members in maintaining Medicaid eligibility.
• Collaborate with Medical Director/Geriatrician/Care Coordinator to ensure holistic service delivery.
