Job summary

Humana is seeking LTSS Care Coordinators (Care Coach 1) in Illinois to assess member needs and guide families toward resources for optimal wellness. The role involves frequent home visits, coordination with care teams, and navigating Medicaid/HCBS interactions to support member goals.

Travel will be 75–90% in the field; the schedule is Monday–Friday; TB screening and driver eligibility are part of the program. This remote position may require occasional office visits.

Qualifications

• One (1) or more years of experience in health care and case management.
• One (1) or more years of experience working with Medicare and Medicaid recipients, long-term care services, HCBS, and managed care organizations.
• Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook.
• Ability to use a variety of electronic information applications/software programs, including electronic medical records.

Responsibilities

• Visit Medicaid members in homes, ALFs, and long-term care facilities; 75–90% travel.
• Assess care needs to ensure services in the least restrictive setting.
• Identify and resolve barriers hindering care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the care plan against member goals.
• Monitor patient care to ensure progress toward outcomes.
• Guide members and families to resources and interact with providers and payer sources as needed.
