Job summary

Humana is seeking LTSS Care Coordinators (Care Coach 1) in Illinois to assess members’ needs and guide families toward appropriate resources. The role requires extensive field visits (75–90% travel) and coordination with care teams to ensure services align with each member’s goals.

You will work in a field setting, travel within the region, and participate in Humana’s TB and driver safety programs. Monday–Friday schedule with 40 hours weekly.

Qualifications

• One (1) or more years of experience in health care and case management.
• Experience with Medicare and Medicaid recipients, long‑term care services, HCBS, and managed care.
• Proficient computer skills: Word, Excel, and Outlook; able to use EMR systems.

Responsibilities

• Visit Medicaid members in homes, ALFs, and LTC facilities (75–90% travel).
• Assess and coordinate care to meet member needs and goals.
• Identify barriers and plan interventions to support optimal wellbeing.
