Job summary

Humana Inc in Columbia, SC is hiring a Care Coach 2 to support South Carolina Medicaid members by assessing health and social needs, building individualized plans, coordinating services, and helping members access resources for optimal health and independence.

This remote, field-based position requires visits to members in homes and facilities, meticulous documentation, and collaboration with the care management team. Travel up to 50% is expected.

Qualifications

• Bachelor's degree in a health and human services field.
• Two or more years of relevant experience in care coordination, case management, health and human services, or a related field.
• Experience assessing needs, developing service or care plans, coordinating resources, and documenting member interactions and outcomes.
• Intermediate to advanced proficiency with Microsoft Word, Excel, and Outlook.
• Ability to travel to meet with South Carolina Medicaid members in homes, assisted living facilities, long-term care facilities, and other care or community settings.
• TB screening is required as part of Humana's Tuberculosis screening program.

Responsibilities

• Conduct telephonic and face-to-face assessments to evaluate health, functional, environmental, psychosocial, and support needs.
• Develop and maintain individualized, member-centered care plans reflecting needs and goals.
• Plan and implement interventions, coordinate services, and monitor progress using data and conversations.
• Identify and resolve barriers hindering engagement, transitions, or outcomes.
• Guide members and families to clinical, community, social, and payer resources, and coordinate with providers and teams.
• Visit members in homes and care settings across the service area and document interactions and outcomes.
• Document assessments, care plans, interventions, member interactions, referrals, and outcomes accurately in required systems.
