Job summary

Humana Inc. seeks a Care Coach to evaluate member needs, coordinate care, and drive interventions across Medicare Advantage populations. You will collaborate with members, caregivers, providers, and internal teams to close care gaps and promote adherence to treatment plans.

The role is remote with training and schedule details provided. Expect telephonic outreach, data review, and documentation across multiple systems to support improved health outcomes and member experience.

Qualifications

• Associate degree is required.
• At least two years of direct adult patient care experience.
• Experience collaborating with healthcare team.
• Strong computer proficiency and ability to learn multiple software applications including EMR.

Responsibilities

• Engage members in quality initiatives, including DSF and FMC.
• Conduct proactive telephonic outreach to members.
• Review data and complete structured assessments to identify needs.
• Educate members about preventive care and connect with resources.
• Collaborate with care managers, providers, and partners to resolve barriers.
• Document assessments and outcomes in applicable systems.
• Follow program, regulatory, and documentation requirements.
• Navigate multiple computer systems in a fast-paced environment.
• Adapt communication to diverse populations and changing business needs.
