Job summary

Qualifications

• Associate degree in any field.
• At least two (2) years of direct adult patient care experience in healthcare settings.
• Experience collaborating with a healthcare team.
• Strong computer proficiency and ability to learn multiple software applications, including EMR.

Responsibilities

• Engage members in Stars and quality initiatives to improve outcomes.
• Conduct proactive telephonic outreach to members using approved channels.
• Review data and complete structured assessments of needs.
• Educate members about preventive care and connect to resources.
• Collaborate with care managers, providers, and partners to resolve barriers.
