Job summary

Humana Inc. is hiring a Care Coach to evaluate member needs, coordinate care, and drive interventions that improve health outcomes and member experience. This remote role involves collaborating with members, caregivers, and providers across Humana's Medicare Advantage populations.

You will engage in telephonic outreach, review data, and educate members on preventive care while navigating multiple systems in a fast-paced, remote environment.

Qualifications

• Associate degree in any field.
• At least two (2) years of direct adult patient care experience.
• Experience collaborating with a healthcare team.
• Strong computer proficiency and ability to learn multiple software applications, including EMRs.

Responsibilities

• Engage members in Stars and quality initiatives to improve outcomes and patient experience.
• Support multiple Stars programs and adapt to evolving needs.
• Conduct proactive telephonic outreach via autodialer and approved channels.
• Review data and complete structured assessments to identify clinical, financial, functional, and social needs.
• Educate members about preventive care and connect them with healthcare and social resources.
• Collaborate with care managers and providers to resolve barriers and promote independence.
• Document assessments, outreach, and outcomes accurately in systems.
• Follow program, regulatory, compliance, and documentation requirements.
• Navigate multiple computer systems and adapt workflows for diverse populations.
