Qualifications

• Associate degree in any field.
• At least two years of direct adult patient care experience.
• Experience collaborating with a healthcare team.
• Strong computer proficiency and ability to learn EMR and other systems.

Responsabilités

• Engage members by phone, using autodialer and approved channels.
• Identify clinical, financial, functional, and social needs from assessments.
• Educate members about preventive care and connect to resources.
• Document assessments and outcomes in applicable systems accurately.
• Follow program, regulatory, and compliance requirements.
• Navigate multiple computer systems in a fast-paced environment.
