Job summary

Humana Inc. is seeking a Care Coach to evaluate member needs, coordinate care, and drive interventions for improved health outcomes and member experience. This remote role involves telephonic engagement and collaboration with care teams to resolve barriers and promote adherence to treatment plans.

You will support Stars programs, document outcomes, and adapt to evolving campaigns while working 40 hours per week.

Qualifications

• Associate degree required.
• Two years of direct adult patient care experience required.
• Experience collaborating with healthcare team.
• Strong computer proficiency and ability to learn multiple software applications, including EMR.

Responsibilities

• Engage members by phone to address care needs and promote adherence to plans.
• Document assessments and outcomes in applicable systems.
• Coordinate with care managers, providers, and partners to resolve barriers to care.
• Navigate multiple systems in a fast-paced environment.
