Humana Inc. is seeking a Care Coach to evaluate member needs, coordinate care, and drive interventions that improve health outcomes and member experience. You will collaborate with members, caregivers, providers, community organizations, and internal partners to identify barriers and promote adherence to treatment plans across Humana's Medicare Advantage populations.

The role is remote with work-at-home requirements and a Monday–Friday schedule after training. Training runs 8:30 a.m.–5:00 p.m.

Qualifications

• Associate degree required in any field.
• At least two years of direct adult patient care experience in settings like long-term care or home health.
• Experience collaborating with a healthcare team.
• Strong computer proficiency and ability to learn and navigate multiple EMR software applications.

Responsabilités

• Engage with members via telephonic outreach to assess needs.
• Coordinate care and connect members with providers and community resources.
• Document assessments and outcomes accurately in Humana's systems.
• Support quality initiatives and performance programs (Stars) as needed.
• Navigate multiple computer systems in a fast-paced environment.
