Humana Inc. is seeking a Care Coach to evaluate member needs, coordinate care, and drive interventions that support improved health outcomes for Medicare Advantage members. This remote role involves proactive outreach, collaboration with a care team and providers, and documenting outcomes in our systems.

You will work from home, with training provided and a Monday–Friday schedule after training. Associate degree and 2+ years' adult patient care are required, along with strong computer and EMR

Formación

• Associate degree required.
• Direct adult patient care experience of 2+ years.
• Experience collaborating with healthcare team.
• Strong computer proficiency and EMR navigation.

Responsabilidades

• Conduct proactive telephonic outreach to members.
• Review data and complete structured assessments to identify needs.
• Educate members about preventive care and connect to resources.
• Collaborate with care managers and providers to resolve barriers.
• Document assessments, outreach, and outcomes in systems.
• Navigate multiple computer systems in a fast-paced environment.
