Job summary

Humana Inc. seeks a Care Coach to evaluate member needs, coordinate care, and drive interventions that improve health outcomes and member experience across Medicare Advantage populations.

You will collaborate with members, caregivers, providers, and internal partners to resolve barriers to care and promote treatment adherence. Training is mandatory for four weeks, 8:30 a.m.–5:00 p.m. ET, with a 40-hour workweek and later 10:00 a.m.–8:00 p.m. ET schedule.

Qualifications

• Associate degree required.
• At least 2 years direct adult patient care experience.
• Experience collaborating with a healthcare team.
• Strong computer proficiency and ability to learn multiple software applications including EMR.

Responsibilities

• Engage members to identify care needs and barriers to care.
• Coordinate care and drive interventions across Humana’s Medicare Advantage populations.
• Conduct proactive telephonic outreach to members using approved channels.
• Document assessments and outcomes accurately in systems.
• Follow program, regulatory, and documentation requirements.
