Job summary

Humana Inc. is seeking a Care Coach to evaluate member needs, coordinate care, and drive interventions across Medicare Advantage populations. You will collaborate with members, caregivers, providers, and internal partners to resolve barriers to care and promote adherence to treatment plans.

The role supports a remote, work-at-home schedule with mandatory training, and offers a 40-hour work week with a pay range of $53,700 to $72,600 per year. Travel to Humana offices may be required occasionally.

Qualifications

• Associate degree required with direct adult patient care experience (2+ years).
• Experience collaborating with healthcare team members.
• Strong computer proficiency and ability to learn multiple software applications, including EMR.

Responsibilities

• Evaluate member needs and coordinate care to support improved health outcomes.
• Engage with members via telephonic outreach, using approved communication channels.
• Document assessments, outreach, and outcomes in applicable systems accurately and timely.
• Navigate multiple computer systems while supporting member-focused care in a fast-paced environment.
• Promote adherence to treatment plans and connect members with healthcare and social resources.
