Job summary

Seamless Assist is hiring a full-time, remote clinical pharmacist to lead GDMT optimization for high-risk cardiac patients, partnering with RN Care Managers and cardiology providers to ensure correct therapies and titration.

You will conduct telephonic reviews, educate patients about benefits and side effects, and document care decisions in the EMR, contributing to value-based care outcomes. A Texas license is required at hire, with a flexible remote schedule and W‑2 employment.

Qualifications

• PharmD degree with active Texas license.
• 3–5+ years of clinical pharmacy with cardiology focus.
• Familiarity with ACC/AHA GDMT guidelines.
• Strong telephonic patient communication skills.

Responsibilities

• Review GDMT optimization and titration for cardiology patients.
• Collaborate with RN Care Managers and cardiology providers.
• Conduct telephonic medication reviews and patient education.
