Seamless Assist is hiring a full-time, remote Clinical Pharmacist with cardiology experience to support GDMT optimization within value-based care programs. This telehealth role collaborates with nurses and cardiology providers to titrate therapies and improve adherence.

You will work in a nurse-led care model, provide telephonic education, and document in the EMR. Texas license and 3–5 years cardio experience required; strong communication and AI fluency preferred.

Qualificações

• PharmD degree and active Texas license in good standing.
• 3–5+ years of clinical pharmacy with cardiology focus.
• Hands-on GDMT experience, HFrEF therapy titration.
• Knowledge of ACC/AHA guidelines.
• Strong telephonic patient communication skills.

Responsabilidades

• Review cardiology medication profiles and optimize GDMT per guidelines.
• Lead titration and dose optimization in collaboration with clinicians.
• Engage patients via telephonic education; reinforce adherence.
• Collaborate with nurses, care managers and cardiologists.
• Document recommendations and interactions in the EMR.
