Job summary

Access TLC is seeking a BSW Social Worker/Case Manager to support HCBA participants in accessing healthcare and long-term supports. The role requires strong communication, organization and documentation skills, and work with a multidisciplinary team to coordinate services in participants' community settings.

The position, though remote, includes regular field visits to participant homes and community locations.

Qualifications

• BSW or related degree (Social Work, Psychology, Counseling, Rehabilitation, Gerontology or Sociology)
• Strong organizational and time-management skills with attention to detail
• Experience in social services, case management, care coordination, healthcare, or related field preferred
• Proficiency with Microsoft Office and learning EHR/case management systems

Responsibilities

• Collaborate with nurses, staff and management to ensure DHCS compliance and HCBA program requirements
• Engage HCBA participants, families and providers to educate, support, advocate and coordinate resources
• Conduct psychosocial and needs-based assessments and identify barriers to health and independence
• Develop and update individualized goals and connect participants with community services
• Coordinate with healthcare and social service agencies to enable timely services
• Maintain case notes, assessments and documentation in MedCompass
• Monitor changes in participant needs and communicate concerns to the care team
• Complete APS referrals and other mandated reports when required
