Job summary

UTHealth Houston Department of Psychiatry & Behavioral Sciences seeks a LMSW-eligible Social Worker to provide biopsychosocial assessments, care planning, and coordination for patients in the TCHATT program. The role emphasizes linkages to long-term care and community resources, documentation across systems, and collaboration with a multidisciplinary team.

Responsibilities include conducting assessments, developing individualized care plans, and educating staff on case management.

Qualifications

• Bachelor's Degree in Social Work or related field required.
• 1 year of social work experience required; medical social work experience preferred.
• Licensed Baccalaureate Social Worker (LBSW) by Texas required; LMSW preferred.

Responsibilities

• Conduct comprehensive social work assessments and develop care plans.
• Coordinate referrals and transitions for patient care within TCHATT program.
• Educate staff on case management processes and psychosocial factors affecting care.
