Job summary

Healspace LLC / Talbot Health Services is seeking a Case Manager (LSW required) to support individuals with behavioral health and co-occurring needs. You will perform biopsychosocial assessments, develop service plans, and coordinate care with a multidisciplinary team in Toledo, OH.

Responsibilities include linking clients to housing, employment, and community resources, advocating for their needs, and ensuring compliance with regulatory standards.

Qualifications

• Active Ohio LSW credential in good standing.
• Bachelor's Degree in Social Work from an accredited college.
• Knowledge of behavioral health, substance use disorders, and recovery principles.
• Strong assessment, case management, advocacy, documentation, and communication skills.
• Ability to maintain professional boundaries with clients.
• Valid Ohio driver's license and reliable transportation.

Responsibilities

• Conduct biopsychosocial and needs-based assessments.
• Develop, implement, monitor, and update individualized service plans.
• Coordinate referrals and linkage to community resources and providers.
• Advocate for clients to promote self-sufficiency and recovery.
• Assist clients in navigating healthcare, social services, and housing systems.
• Monitor treatment participation and collaborate with providers on progress.
• Participate in multidisciplinary staffing meetings and team discussions.
• Maintain timely, accurate documentation per regulatory requirements.
• Assist with discharge planning and aftercare coordination.
