Job summary

CareTalk Health is a virtual medical practice seeking a Bilingual Registered Nurse (Spanish/English) to join our remote care team. The role blends care management, care navigation, and AWV chart preparation to support providers in delivering comprehensive, compliant, patient-centered care.

Qualifications

• Bilingual in English and Spanish; able to conduct patient conversations in both languages.
• Active, unrestricted RN license in a compact state; state licenses may be added by the company.
• Minimum 2–3 years of clinical nursing experience; telehealth or virtual care experience preferred.
• Experience working remotely and using EHR/telehealth platforms.
• Proficiency with CCM, RPM, RTM, and APCM program workflows is a plus.

Responsibilities

• Prepare AWV charts and ensure CMS/compliance-ready documentation.
• Provide telehealth-based care management and care coordination across programs.
• Close care gaps, coordinate orders/referrals, and monitor patient progress.
• Document patient status and updates for primary care teams and specialists.
• Maintain HIPAA-compliant records and adapt to patient-centered, culturally competent care.
