Job summary

CareTalk Health is seeking an experienced Registered Nurse to join our virtual care team in a fully remote capacity. You will perform AWV chart preparation, care management, and care navigation to support providers delivering comprehensive, evidence-based care across CCM, RPM, RTM, and APCM programs.

Qualifications

• Active, unrestricted RN license in a compact state.
• Minimum 2–3 years of clinical nursing experience.
• Experience in telehealth or virtual care settings.
• Experience working remote.

Responsibilities

• Prepare AWV charts to support CMS compliance and provider efficiency.
• Review patient histories and medications during virtual visits.
• Identify overdue screenings and care gaps prior to visits.
• Communicate summaries and key findings to providers before AWV.
• Coordinate with patients to obtain missing information as needed.
• Ensure documentation meets Medicare, CMS, and organizational requirements.
• Provide telephonic and portal-based care management for complex patients.
• Develop, implement, and monitor individualized care plans across CCM, RPM, RTM, APCM.
• Use motivational interviewing to engage patients and caregivers.
• Monitor progress and adjust care plans as needed.
