Job summary

SCAN Health Insurance is seeking a Registered Nurse (RN) – Care Management to deliver high-touch, person-centered care management for members with complex medical needs. Responsibilities include in-home and telephonic assessments, care plan development, education for members and caregivers, and coordination with an interdisciplinary team to stabilize risk and improve outcomes.

Qualifications

• ASN required as nursing qualification.
• BSN preferred.
• Graduate or Advanced Degree preferred.
• CA Registered Nurse (RN) required.
• CCM certification strongly preferred.
• BILINGUAL English/Spanish required.
• Travel 50%+ in assigned area; valid driver's license and reliable transportation.
• 3+ years clinical experience in care management or related field.
• 2+ years working with seniors; hybrid remote strongly preferred.

Responsibilities

• Conduct comprehensive clinical, functional, and environmental assessments in homes and via telephonic encounters.
• Coordinate with interdisciplinary team to address barriers to health and support coordinated, member-centered care.
• Develop and update individualized care plans with measurable goals.
• Provide disease-specific education, coaching, and self-management support.
• Support medication safety through reconciliation and coordination with providers.
• Coordinate care across primary care, specialists, hospitals, SNFs, home health and community resources.
• Document care plans and progress notes within established timeframes.
• Manage transitions of care after ED visits, hospitalizations or discharges.
• Comply with CMS, DHCS, and related regulatory standards.
• Perform CICM activities for Populations of Focus.
