Job summary

Salvo Health is hiring rn-led care professionals to deliver disease management and health education through a remote patient monitoring program. The role emphasizes bilingual Spanish communication, patient education and collaborative care with a multidisciplinary team.

The successful candidate will coordinate with clinicians, create personalized action plans, and engage patients regularly to drive behavior change and improved health outcomes. Prior telehealth experience is a plus.

Qualifications

• Active RN license in the United States.
• Bilingual (Spanish speaking, reading, and writing) required.
• 2+ years as Registered Nurse.
• 2+ years in patient-facing or customer-facing roles.
• Excellent written and verbal communication skills.
• Familiarity with medical terminology and general medical office procedures.
• Experience with telehealth or healthcare startups preferred.

Responsibilities

• Provide exceptional care, disease management and health education to patients.
• Support goal setting for individual patients asynchronously.
• Create personalized action plans with guidelines to reduce or eliminate unwanted behaviors.
• Help clients understand motivations and create behavior change plans.
• Conduct regular outreach to patients to support engagement and elicit behavior change.
• Coordinate with clinical team members to provide an exceptional patient experience.
• Develop professional working relationships with patients and team members.
• Create and distribute health education materials.
