Included Health in California is seeking a telephonic Nurse Care Manager to guide members through complex medical and behavioral health journeys, coordinating with a diverse clinical team to deliver integrated remote care.

The ideal candidate holds a CA BSN, 5+ years nursing, 2+ years in care management, and an active CA RN license; remote work is available, Spanish desirable, and commitment to HIPAA and empathetic patient support is essential.

Qualifications

• BSN required for clinical leadership and care planning.
• 5+ years of nursing experience.
• 2+ years in care management experience.
• Active California RN license is required.
• Willingness to obtain licensure in multiple states.

Responsibilities

• Deliver telephonic/virtual care management to members.
• Develop care plans with team and members to meet goals.
• Help members navigate medical conditions, treatments, and benefits.
• Coordinate with local providers for integrated care.
• Provide longitudinal follow-up and empathetic support.
• Assist during acute episodes with coordinated case management.
• Coordinate resources addressing clinical and social needs.
