Job summary

Medix is seeking a Remote Telephonic RN Case Manager to provide telephonic case management for chronically ill members within a C-SNP population. The role requires an active Compact State RN license or CA license and a strong commitment to patient advocacy and care coordination.

This high-volume, phone-based position involves 30–40 calls per day, working 8:00 AM–5:00 PM PST, Monday through Friday. Fluency in English and Spanish is required, and a dedicated home office is essential for

Qualifications

• Active, current RN license in a Compact State and/or California.
• Minimum of 1 year of dedicated case management experience.
• Fully bilingual in English and Spanish (verbal and written).
• Strong computer proficiency and accurate real-time note-taking.

Responsibilities

• Provide telephonic case management for complex chronically ill members; assess and develop individualized care plans.
• Deliver clinical and disease-state education (e.g., CHF, A1C/diabetes management, labs, medications).
• Coordinate care with internal/external partners; present cases in ICT/IDT meetings.
• Identify barriers to care and advocate to close gaps in services.
• Delegate non-clinical tasks (transportation, scheduling, referrals) to Care Coordinators.
• Document all clinical notes and care plans in the management system in a timely manner.
• Manage a high-volume call queue with ~80% outbound and 20% inbound calls.
