Job summary

Alignment Health is seeking a Remote RN Case Manager SNP in California. You will coordinate care for members, work with families and physicians, and ensure timely access to needed services within the benefit structure.

The role requires a California RN license (Non-Compact) and at least 2 years of clinical experience plus 1 year of case management, with opportunities for growth in a fast-growing team. Expect 8:00am–5:00pm Pacific Time schedules.

Qualifications

• Minimum 2 years' clinical experience.
• Minimum 1 year case management experience.
• RN licensure in California (Non-Compact) required.
• AS in Nursing required; BSN preferred.

Responsibilities

• Coordinate care as a resource for member, family, and physician.
• Facilitate referrals/authorizations within benefit structure.
• Perform comprehensive assessments within scope of practice.
• Collaborate with member, PCP, and care team on plan of care.
• Interface with PCPs, Hospitalists, and specialists on care plans.
• Provide education and self-management support.
• Resolve problems with providers, claims, or services.
• Coordinate with delegated providers to ensure coordination.
