Concert Health is hiring a Collaborative Care Clinician I in a 100% remote position. The role reports to the Clinical Excellence Manager and delivers evidence-based interventions within the Collaborative Care Model for pediatric and adult patients with mild to moderate symptoms.

The job requires a Master’s in Social Work or related field and MA licensure (LCSW/LICSW), plus cross-licensing to PA. Salary ranges vary by supervision needs and language skills, with biweekly pay and Eastern Time

Qualifications

• Master’s Degree in Social Work, Counseling, or related field from an accredited program.
• Licensed to practice in the state of Massachusetts as a clinician (LCSW, LICSW or equivalent).
• Ability to complete cross-licensing to the state of Pennsylvania.
• Minimum 1–5 years experience as a licensed professional.
• Secure, high-speed internet connection for video conferencing.

Responsabilités

• Provides care to patients, based on needs of assigned Care Pod, customizing treatment plan to individual patient needs.
• Coordinates behavioral health care with the patient’s primary care provider and other service providers.
• Screens patients for behavioral health and substance use disorders using PHQ9, GAD7, or similar tools.
• Monitors symptoms and treatment side effects with goal of symptom reduction.
• Supports psychotropic medication management prescribed by PCPs and adherence.
