Concert Health is seeking a licensed Behavioral Health Clinician to deliver evidence-based interventions within the Collaborative Care Model for pediatric or adult patients with mild to moderate symptoms.

You will coordinate care with primary care teams, document progress in Salesforce, and participate in supervision to ensure optimal patient outcomes. The role emphasizes collaboration, documentation, and family-centered care.

Qualifications

• Master's degree in social work or related field required.
• Licensed to practice in MA as LCSW/LICSW or equivalent.
• Ability to cross-license to PA.
• 1–5 years experience as a licensed professional.
• A secure, high-speed internet connection capable of supporting video conferencing.
• Proficient in medical interpretation and documentation in English and Spanish that meet industry standards.

Responsabilités

• Provides care to patients, based on needs of assigned Care Pod, customizing treatment plan to individual patient needs.
• Supports and coordinates behavioral health care with the patient’s PCP and other providers.
• Screens patients for behavioral health and substance use disorders using PHQ9, GAD7, and other tools.
• Monitors symptoms and treatment side effects to reduce symptoms.
• Supports psychotropic medication management in coordination with PCPs.
• Tracks patient follow-up and clinical outcomes using a registry.
