Job summary

Concert Health is hiring a Collaborative Care Clinician I for a 100% remote role in the United States. This associate-level clinician collaborates with PCPs and supports evidence-based behavioral health care for patients across age groups using the Collaborative Care Model.

The role emphasizes supervision, ongoing training, and a strong focus on patient outcomes. Qualifications include a Master’s in Social Work or Counseling and licensure in Washington, with 1–5 years of experience.

Qualifications

• Master's degree in Social Work or Counseling required.
• Licensed in Washington as ASW/APCC or equivalent OR licensed independent clinician (LCSW/LPCC) in Washington.
• Minimum 1–5 years of licensed professional experience.

Responsibilities

• Deliver care via the Collaborative Care Model to patients (pediatric and/or adult).
• Coordinate behavioral health care with primary care providers and other services.
• Screen for behavioral health and substance use disorders using standardized tools.
• Monitor clinical symptoms and treatment side effects to reduce symptoms.
• Support psychotropic medication management under PCP supervision.
• Document patient progress in the EHR and Salesforce systems.
• Create mutual care plans with patients and care teams.
• Adjust care plans with supervisor input when needed.
• Participate in caseload consultations and supervision sessions.
• Identify social determinants barriers and assist in care navigation.
