Concert Health is hiring a Collaborative Care Clinician I to deliver evidence-based interventions under the Collaborative Care Model for pediatric and adult patients with mild to moderate symptoms.

You will work remotely with supervision, coordinating with PCPs and care teams to promote symptom reduction and better patient outcomes. Requires MA licensure and cross-licensing where needed; bilingual English/Spanish is valued.

Qualifications

• Master’s Degree in Social Work, Counseling, or related field.
• Licensed to practice in Massachusetts as a clinician (LCSW, LICSW or equivalent).
• Ability to cross-licensure to Pennsylvania.
• 1–5 years of licensed clinical experience.
• Secure high-speed internet for video and cloud-based systems.
• Proficient in medical interpretation and documentation in English and Spanish.

Responsabilités

• Provide care to patients based on assigned Care Pod with customized treatment plans.
• Coordinate behavioral health care with patients’ PCP and other providers.
• Screen patients for behavioral health and substance use disorders using PHQ9, GAD7, CSSRS, or similar tools.
• Monitor symptoms and treatment side effects to reduce symptoms.
• Support medication management and assess adherence and side effects.
• Track patient outcomes using a registry and document encounters.
• Plan mutual self-management goals and interventions with patients.
• Adjust care plans and referrals as needed for non-improvement.
• Document progress in Salesforce and EHR for sharing with care team.
• Complete relapse prevention plan for patients in remission.
• Educate patients about behavioral health and treatment options.
• Set up care plans with the patient and care team.
• Participate in caseload consultations and supervision.
• Identify social determinants and connect to appropriate care.
