Job summary

• Master’s degree in social work, counseling, or related field.

Responsibilities

• Provide care based on Care Pod needs and individualize treatment plans.
• Coordinate behavioral health with the patient’s PCP and other providers.
• Screen for behavioral health and substance use disorders using standard tools.
