Job summary

Concert Health is hiring a Collaborative Care Clinician II for a 100% remote role. You will provide evidence-based behavioral health interventions using the Collaborative Care Model to pediatric and/or adult patients with mild to moderate symptoms.

The role reports to Clinical Excellence Manager and requires an IL independent license, 3–5 years experience, and proficiency in tools like EHRs and Salesforce. Join a diverse team focused on integrated care.

Qualifications

• Master’s degree in Social Work, Counseling, or related field.
• Licensed to practice in Illinois as an independent clinician (LCSW/LICSW, LMHC, LMFT, LPC, or equivalent).
• Minimum 3–5 years experience as a licensed professional.
• High-speed internet connection capable of supporting video conferencing and cloud-based systems.

Responsibilities

• Provide care to patients based on needs of Care Pod; customize treatment plans.
• Coordinate behavioral health care with the patient’s PC provider and other services.
• Screen patients for behavioral health and substance use disorders using PHQ9, GAD7, etc.
• Monitor symptoms and treatment side effects to reduce symptoms.
• Support psychotropic medication management prescribed by PCPs.
• Document patient progress in Salesforce and EHR for sharing with care team.
• Establish mutually agreeable care plans with patients and families.
• Participate in supervision and caseload consultations with clinicians.
