Job summary

Concert Health is seeking a Collaborative Care Clinician I for a fully remote role in the United States, reporting to Clinical Excellence Manager. You will deliver evidence-based behavioral health interventions within the Collaborative Care Model to pediatric and/or adult clients, aiming for symptom reduction and strong outcomes.

You will collaborate with PCPs, conduct screenings, manage treatment plans, and document progress in Salesforce and the EHR, while participating in supervision and care

Qualifications

• Master’s Degree in Social Work, Counseling, or related field.
• Licensed in Washington state: ASW/APCC or equivalent OR independently licensed (LCSW/LPCC or equivalent).
• 1–5 years of licensed professional experience.
• A secure, high-speed internet connection for video conferencing and cloud systems.

Responsibilities

• Provide care to patients per Care Pod needs with customized treatment plans.
• Coordinate behavioral health care with PCPs and other providers.
• Screen for behavioral health and substance use disorders using PHQ9, GAD7, CSSRS, etc.
• Monitor symptoms and treatment side effects toward symptom reduction.
• Support medication management and treatment adherence with PCPs.
• Document progress in Salesforce and EHR for sharing with care team.
• Implement relapse prevention plans and educate patients on options.
• Engage in regular supervision and caseload consultations with psychiatric consultants.
