SCAN Group in Long Beach, CA, is seeking a remote Care Coordination Specialist to support SCAN members with holistic care planning, referrals, and system navigation. The role emphasizes compassionate engagement and accurate documentation across multiple systems.

The position requires bilingual English and Spanish or Vietnamese, or Cantonese or Mandarin, and at least 1 year in healthcare/customer service. An annual bonus, PTO, and wellness programs are included.

Qualifications

• High School Diploma required. Some college preferred.
• Bilingual requirement with test for skills.
• 1+ year healthcare/customer service experience.
• Basic technical and organizational skills.
• Strong communication and confidentiality abilities.
• Knowledge of CMS and DHCS regulations.

Responsibilities

• Contribute to care and well‑being of SCAN members by assessing, planning, coordinating, and monitoring holistic needs.
• Address care coordination needs, referrals, and navigating healthcare system.
• Perform assessments and screenings to determine interventions and referrals.
• Triages contact with members, providers, and medical groups for benefits and access issues.
• Document assessments, care plans, and notes in multiple systems within required timeframes.
• Demonstrate critical thinking to resolve issues with members, providers, and vendors.
• Coordinate care with county offices and governmental agencies as needed.
• Maintain professionalism and empathy in all interactions.
